We have been blessed with a very busy practice and in order to do the job well, We can not accommodate new patients at this time.
Therefore, the following information is for established patients only.
CANCELLATIONS AND LATE ARRIVALS
We reserve your time slot especially for you!!!!! As such, we appreciate your prompt notice if you are unable to keep your appointment or you anticipate being late. Your early notice allows us to adjust our schedule and maintain punctuality. Ideally, we ask that you give us at least 24 hours notice for cancellations; that way, the staff can offer the time slot to another patient. If you are running late, the staff will need to check with the schedule and see if it allows you to be seen or if you will need to reschedule. Sometimes it works out.
No shows or missed appointments are costly. If you do NOT notify us of your cancellation we reserve the right to charge $25.00 for regular appointments and $50.00 for no-show physicals.
This is a special opportunity for patients to work in a collaborative fashion with other patients who have the same chronic condition such as addiction, hypertension, diabetes, high cholesterol, menopause, osteoporosis, anxiety or others. Many insurance companies cover these visits. They are usually 2 hours long and include a topic of discussion, such as new medication. Most groups have about 8 – 10 participants. Each member gets checked in like a “regular” office visit and has a chance to meet with the practitioner one-on-one, privately at the end, if necessary. Research has shown that the benefit comes from the sharing of experiences and support that the individuals bring to the group as well as more time devoted to prevention and education on the part of the doctor and the resources the doctor can bring to the group such as dietician, physical therapist, etc.
This is NOT mandatory if you have a chronic condition and it will take us some time to set these up. My hope is that IF you are interested you will sign up with our receptionist or on-line and we can contact you to get started.
AS A COURTESY WE ATTEMPT TO CALL THE DAY BEFORE YOUR APPOINTMENT TO REMIND YOU, BUT IT IS YOUR RESPONSIBILITY TO REMEMBER THE APPOINTMENT YOU HAVE MADE WITH OUR OFFICE. PLEASE MAKE SURE WE HAVE YOUR MOST UPDATED INFORMATION SO THAT WE CAN REACH YOU!
Scheduled nursing visits are also available during regular hours as well
WHAT TO BRING TO YOUR APPOINTMENTS
Please bring your insurance cards to each appointment. Bring an updated list of all your prescriptions (from ALL your prescribing doctors) OR the actual bottles themselves AND a list of any over-the counter products such as aspirin, vitamins, supplements.
Please check with me before transferring your medical records from another office. Most offices charge for this service and it is often unnecessary to transfer your entire record. Sometimes arrangements can be made electronically
AFTER HOURS COVERAGE AND EMERGENCIES
Simply the most time efficient way to refill your medication is by contacting your pharmacy and having them generate the request through the latest, secure, web-based system that communicates directly with your electronic chart in our office. We can respond quickly and accurately by electronic means.
It is a good idea to give yourself about 3 or 4 days safety net on refills especially if there is a weekend or holiday.
If you were previously advised to have lab-work or a recheck in the office before your medication refills expired, we will only refill the medication for a limited time in order for you to schedule an appointment in the office.
SCHEDULE II or “TRIPLICATES”: Please note that the state of California does NOT recognize refills for these prescriptions. You will need to pick a new one up monthly for those patients in need of ongoing medication. Furthermore, regular visits will be necessary to evaluate the safety and necessity of these medications. Patients on regular use of schedule II narcotics will be asked to sign a pain medication contract.

We are thrilled with our electronic medical record system ELATION. It has a fantastic patient portal called Passport. Be sure to get invited when you complete your check in at the office
I feel that patients should be notified and given the results of ALL the tests that are ordered through our office regardless of whether the findings are normal or abnormal. If you have undergone medical testing (including blood tests, PAP smear, X-rays, etc) and have not received the results within 14 business days please notify our office either by telephone or by e-mailing reception. While we make every effort to ensure that the results of all the tests are reported back to our office, there are times when they simply don’t arrive (wrong patient information, broken sample, simple human error). Therefore we ask that you, as the patient, take the ultimate responsibility in ensuring that the results of all completed tests are reported back to you.
